Spinal Meningitis
Clinical Profile: Young patient with quadriparesis, low grade fever and weakness worsened after delivery
Imaging: CEMR shows diffuse
leptomeningeal enhancement involving the cervicodorsal cord with plaque like
enhancement over the spinal cord. There
is evidence of prominent CSF space in the
upper cervical anteriorly which shows surrounding enhancement and
displacement /thinning of cervical cord posteriorly. Cervicomedullary junction appears
distorted. There is cord swelling
involving the lower cervical and dorsal cord with prominent central T2
hyperintense signal.
These
findings possibly indicate ?tubercular meningitis-brain and spine with
arachnoiditis and dilated anterior CSF space, with associated spinal cord edema
with syringomyelia. Needs clinical and
CSF correlation.
